This is the right place to get the export permit for your pets.

Apart from the bank payments, they ask you for two copies of your passport and a copy of the vaccine records. They’ll keep one of the two copies of the health certificate the veterinarian should give you, as well as the vet’s registration with IPSA’s paper (I’d get a copy of that just for your records).

They are kind of slow and arrogant, so be patient.

almost everything as described. Handed in Receipt of the bank payment, copy passport, copy vet medical papers and a copy of our dogs last rabies vaccination. After this he just gave me a post it with a file number and said that's all I need. So hopefully this will work out.

Cetex/Vecun is indeed upstairs in the yellow building behind the blue and white monument. They made photocopies for us but if can bring a copy of your passport and rabies vaccination (or pet passport) you will help the process.
They will give you a package with all needed papers to give to the IPSA at the border.

On the second floor of a little yellow building with a decently sized parking lot.

The woman working asked for the health certificate copy, a copy of my passport, and the receipt showing payment of the Cetrex fee from the bank.

She then typed up a doc with my name and a case number. Told me to give this to IPSA at the border when I go to close things out with them. They’ll look me up by the case number listed.

She also had me write me email down, and she sent me a receipt of the transaction.

these two pins here are for the same building. there is not an IPSA station here. that is found at the border, and is the last stop.

this place was smooth and the gentleman that helped us was friendly. even though we don't speak much English he was very patient.

It’s the yellow plazita second floor. Says Centrex above the door, super small.
Need to have a health certificate from Nicaragua, make two bank transfers, then head there.
I attached a paper the woman there gave me that instructs what we needed.

the location is across the street from these coordinates, in a small yellow shopping center. cetrex, here is the third step (after going to vet and bank, and before going to IPSA) to get exportation papers to Costa Rica for the pets. they ask for 2 copies of the receipt you receive from the bank for cetrex (you can make copies next door), a copy of your passport and original vet health certificate (they give it all back). very straightforward. open 8-5 M-F, with 30 minute lunch break around noon.

IPSA CETREX (centro de trámite de exportación).
It took us a loooong time to find this place because there are other IPSAs but here it’s the only place where they give you the export permit for your pets.
They are open from Monday to Saturday until 5pm, but take a lunch break from 12 to 1 pm.

They’ll ask you for one copy of the vaccine record, two copies of your passport and will keep one of the two health certificates the veterinarian should give you and the paper with the veterinarian registration with IPSA (apart from the bank payments, as of today 150 córdobas for CETREX and 13 USD for IPSA).

Good luck, they are a bit slow and kind of arrogant.
